American Samoa Online Traveler Health Declaration Portal
A traveler entering American Samoa must complete a health declaration. For convenience, traveler can submit responses using this online web portal, or traveler will be required to complete a health declaration form upon arrival in American Samoa. Below is an outline of the steps necessary to complete your online health declaration form.
Complete Form
Fill out the online form for yourself and any additional traveling companions if there are any. Once you have submitted the form successfully an email will be sent to the email address that you provided in the form detailing the next steps to complete your health declaration. Click the link in the email you received.
Login
If it is your first time completing the form it will prompt you to set a password before logging in. If you have previously completed a form you can login with your email and password. If you do not remember your password you can click the forgot password link to reset your password. After resetting your password proceed to log in.
Upload Documents
Once you log in, your account page will display all the previous as well as current declarations you have submitted. Click the “Upload Documents” button on the right of your health declaration that has your intended travel date. You will upload an image of either your passport or state issued ID.
Save QR Code
Once you have uploaded an image of either your passport or state issued ID, you will receive a final email stating you have successfully completed your health declaration. The email will also contain a QR code which you need to present to the health official when you arrive on the island. Please save this QR code.
